Ages and Stages Questionaire

Upon enrollment we send out Ages and Stages Questionnaires (ASQ's). The ASQ is for the parent to take home, fill out and return.  The questionnaire is designed for early intervention and to screen for any developmental delays. 

"The Ages and Stages Questionnaires is a screening tool that can be used to screen infants and young children for delays during the crucial first five years of life. ASQ meets the recommendation by the California Department of Social Services to use standardized tools to identify children for referral."

After enrollment we send ASQ's out annually in August.
